Primary Care Networks were established to enhance the delivery of primary health services. The St. Albert and Sturgeon Board of Directors consists of family physicians and, as such, ensures our programs and services in our member clinics are focused on excellence in patient care and that every patient in the St. Albert and Sturgeon County area has a medical home.

The St. Albert and Sturgeon PCN provides comprehensive and collaborative health care through its 62 family physicians in 18 clinics. Our team of health care practitioners and staff work with patients and other stakeholders through PCN programs and services. We operate as an extension of the affiliated family physicians’ practices.
- Health Management
The PCN Health Management team consists of Nurses and Pharmacists that can assist you to promote wellness and manage chronic disease.
- Nutrition
Our PCN Dietitians are nutrition experts and your trusted source for advice and resources. They’re dedicated to helping you gain knowledge, skill and confidence to achieve the healthiest lifestyle possible.
- Senior’s Health
As we age, we can develop new health concerns. The PCN provides support and education for a variety of seniors topics from exercise and nutrition to memory and falls prevention.
- Mental Health
The PCN Mental Health team consists of Mental Health Nurses that use the Behavioral Health Consultation model to assist you with habits, behaviours, stress, worry, or emotional concerns about physical or other problems that interfere with daily life and/or overall health.
- Maternity
Primary Care Maternity Clinics offer obstetrical patients excellent care during their pregnancies. The goal of these clinics is to provide a service to the patient and their existing family physician for the pregnancy and delivery, including newborn hospital care.
